The Schulman and Schachne Institute’s Adult Day Health Care Program offers health care services and activities to the community for persons who have varying health care needs, enhancing their health status and allowing them to remain in the community. As a community-based long term care program, we provide comprehensive health care to the frail elderly, chronically ill and disabled individuals in a congregate day setting.

The Schulman and Schachne Institute for Nursing and Rehabilitation Adult Day Health Care Program is designed to provide a safe and supportive environment. By offering health care services, activities, education and social stimulation, we enable adults to remain in the community, living at home as long as possible, despite physical or cognitive impairments.

Mission

Services are provided that will bring the clients to the highest level of wellness and independence within their capabilities. Improving the quality of life, maintaining independence, treating each individual with respect and dignity are our highly-valued goals. To achieve this, diagnostic, therapeutic, rehabilitative and/or palliative care services are offered.

Adult Day Health Care is an alternative health care model, not a substitute for needed care. Adult individuals can be maintained in the community with our Adult Day Health Care Program as a support system.

Services

Our experienced staff will develop comprehensive, interdisciplinary care plans to meet the individualized needs of each client. Our services include: Medical and Nursing Services

Health monitoring, hands-on care, and education are the primary goals; all under the supervision of a Nurse and a Medical Director. Social Services

Evaluating and offering services for a person’s psycho-social needs. Case Management services are provided, fostering a linkage with community support systems and encouraging continued independence.

Comprehensive Medical Services Primary Medicine, Dentistry, Podiatry, Psychiatry services are available as well as referrals to other clinical and subspecialty areas.

Rehabilitation Services
Physical, Occupational, and Speech Therapies along with Audiological services and Therapeutic services are offered when ordered by the client’s primary care physician. Socialization and educational activities are also available.

Meals
A continental breakfast, nutritious midday meal, and snacks are prepared daily. (Kosher meals are available upon request.)

Schedule
The Program is open Monday through Saturday from 8:30 a.m. to 4:30 p.m. Days of participation will vary depending on individual needs and circumstances. Flexible hours can be arranged.

Transportation The Program facilitates the provision of door to door transportation by trained drivers in safe handicapped accessible vehicles.

Eligibility
After referral is received, a personal interview will be conducted. Clients will be accepted into the program based upon medical need and the recommendation of a physician.

Eligibility will be determined based on physical and psychological assessments. There may be an accompanying need for rehabilitation services, to restore or maintain optimal level functioning.

Fees Adult Day Health Care is covered under the New York State Medical Program. Private rates are also available for those not eligible for Medicaid.
