Brookdale's Home Healthcare Agency

Home care can enable someone to safely return home after an episode of illness or surgery. Brookdale's Certified Home Healthcare Agency is a hospital-based program able to provide continuity of care and intermittent, skilled care to adult community residents, often after they are discharged from the acute-care environment of the hospital.

Services include:

• Skilled nursing
• Physical, occupational and speech therapies
• Social work services
• Home health aide services

Care must be at a skilled level and certified as medically necessary by a physician and must be limited to intermittent care.

Services are usually covered by insurance, including Medicare, Medicaid and commercial carriers.

Brookdale's Home Healthcare Agency is proud that for two consecutive years the New York State Department of Health has found it to be in full compliance with all standards during the annual survey process.
