Welcome to the Department of Ambulatory Care & Community Health Services at Brookdale University Hospital and Medical Center.

Joan Sclafani
Associate Administrator

The Department of Ambulatory Care & Community Health Services was established in 1971 as a clinical service to provide primary and specialty care to the community surrounding Brookdale University Hospital and Medical Center. In 1994 a decision was made to move primary care out to surrounding communities to encompass a larger service area and a broader patient base including: East Flatbush, Brownsville, Canarsie, Crown Heights and East New York.   The department now oversees The Ambulatory Care Center (ACC) at the main Hospital and six Brookdale Family Care Centers (BFCCs).

The Brookdale Family Care Centers are primary care centers located in the communities of Brownsville, East New York, Canarsie, East Flatbush and Crown Heights.

In addition, The Ambulatory Care Center offers more than 50 specialty and sub specialty clinics in medicine, surgery and OB/GYN.

Under the auspices of the Ambulatory Care Department are the Ambulatory Surgery Unit (Same Day Surgery) and the Endoscopy Suite.
